Surprising guests by hiding things in pastry goes back to Ancient Rome. Ancient Rome was known for gluttonous feasts teeming with tureens of wine and towers of lavish dishes. Wealthy diners in the Middle Ages continued the tradition, often posing their dishes to make the cooked animals come alive. Medieval banquets often featured recipes meant to shock and entertain as much as nourish. They included live eel pie, dead animals made to look alive and live animals made to look dead. Chefs enjoyed splicing together bits of different creatures to make a beast they called a cockentrice.
By the end of the Middle Ages, it was very popular to stuff live birds inside food in order for them to fly out once it was sliced open! The crust was thick and baked first, later placed on top to form a lid, like that of a pot (hence the term “pot pie”). The birds would then be set inside and the lid put on prior to serving. If you’re wondering how someone could bring themselves to consume an avian influenza flavored dish, don’t fret. The chef would usually have another pie on standby to actually eat, as the birds were sure to leave a few surprises of their own in there…
At a banquet hosted by famed French engineer Philippe Le Bon, an enormous meat pie containing a reported twenty-eight musicians, men and children, played as the giant crust was opened. In 1626, the Duke and Duchess of Buckingham presented Charles I with a pie from which emerged a dwarf. Standing just 45 centimeters tall, Sir Jeffery Hudson, a.k.a. “Lord Minimus,” emerged from the crust in a full suit of armor!

Die Entwicklung zur „Pie Girl“: Weibliche Überraschungen für das männliche Publikum
By the 1800s, the humans buried in pastry seemed to be limited to attractive women at decadent parties.

In May 1895, notorious architect Stanford White held a banquet in New York that was reported on in the San Francisco Chronicle and the Cincinnati Inquirer. During the party, an unusually large pie was brought out and, unexpectedly, out popped a scantily clad young girl by the name of Susie Johnson. The pie divided as if by magic, and, falling apart, disclosed Susie Johnson, the sixteen-year-old model. A great bevy of canaries, which had been inclosed with her, flew into the room and perched on the easels, on the pictures, anywhere they could find refuge. Then there was a great shout [...] and the young model was lifted from the table to the floor. She was dressed in filmy black gauze.
On Sunday, a seven-column of Susie capering on the table-most of her nudity blurred-filled the first page of the Sunday World Supplement. And Susie Johnson? She abruptly disappeared. Some sensational newspaper accounts associated Stanford White with the girl’s downfall and disappearance.
Der Wandel im 20. Jahrhundert: Von der „Pie Girl“ zum Showgirl im Kuchen
By the 1950s, showgirls jumping out of giant cakes became mainstream for office gatherings and conventions. By the 1950s, it became downright mainstream for bachelor parties, office wingdings, and conventions to feature an attractive woman in a giant cake-usually in a skimpy bathing suit, or completely nude, depending on the audience and event. The cakes were decidedly real.
Pop out cakes must be large enough to contain a woman in a squatting position, but not too high in order to allow her to gracefully get out of the cake. There are many variants in shapes, sizes and heights. For a more impressive visual effect, some pop out cakes are so tall that the lady can't get out of the cake by herself; in these cases, only the upper part of her body is exhibited, and she is meant to remain inside of the cake for some time after her popping out, until the cake is wheeled offstage later during the party. Some of the most refined pop out cakes are tall multi-tiered constructions in which the upper tiers are designed to fold down inside the bottom tier when the woman pops out, allowing her to exit the cake without difficulty.
Until the beginning of the 1970s most pop out cakes were real. Things changed in 1975 when "the AP newspaper published an interview with a baker that worked making them for a small fee of $2,000. Nowadays, the construction is generally made of corrugated cardboard, often covered with whipped cream or frosting to make them look real. This kind of pop out cakes present the advantages of being cheaper to build and easier to move, due to their lightness, but they are also much more fragile and should be handled with care. At the opposite, expensive pop out cakes can be entirely constituted of real edible cake arranged around the central hole.

Kulturelle Bedeutung und Popkultur
Pop out cakes are a common trope, used especially in television and films. In Game of Thrones, a pop out cake has doves fly out after Joffrey slices the cake with his sword at his wedding. Occasionally a joke of some kind is added, such as an old lady or a hairy man popping out of the cake, in comedies like Family Guy or The Simpsons.
From Some Like It Hot, in which a thug pops out with a machine gun, to more modern examples, like Mariah Carey’s “Loverboy” music video, pop culture keeps the tradition alive.
In “Confessions of a Show Girl: The Truth About Being the Girl Inside the Cake,” Maren Wade tells Las Vegas Weekly the less-than-glamorous, sweaty, and stifling experience she had, crouched inside a Smart Car-sized cake for the birthday of a Fortune 500 CEO in 2014.
